Authorization For Monthly Billing - My Primary Pet Care Plan

I authorize My Primary Pet Care to charge the monthly plan cost to the credit card or bank account I provide. This authorization will remain in effect until I notify My Primary Pet Care in writing at [email protected]. I agree to be billed on the same day of each month ongoing, which will appear as indicated in my initial enrollment. I acknowledge that I have read and understand the applicable rates based on my enrollment, including any additional charges for a spouse and/or dependents.

I authorize AFE to use the information above to initiate an electronic fund transfer from my account or to process the payment as a check transaction or bank drawn draft from my account for the amount of $24.97. If my payment is returned due to insufficient funds, I authorize Dylan Consulting to make a one-time electronic funds transfer or to use a bank draft drawn from my account to collect a fee as allowed by state law.

SMS/TEXT AUTHORIZATION:

I expressly consent to receive and authorize Association for Entrepreneurship USA (“AFE”) or its affiliates, business associates and third-party service providers acting on its behalf to deliver calls and SMS/text and voice messages to my cell phone and residential landline as applicable, possibly using an automatic telephone dialing system and/or using an artificial or pre-recorded voice, irrespective of whether my telephone number appears on any state or federal “Do Not Call” list. I understand that cellular rates may apply based on my data plan. These calls and messages will be for telemarketing, advertising and other purposes including servicing my account. Agreeing to receive such calls is not a condition of purchasing any goods or services.

I am enrolling as a Member in the Association. I agree to abide by the Terms and Conditions of the Association which are attached to this Membership Application. To become a Member, I understand that I must be an individual “Entrepreneur”. An “Individual Entrepreneur” is defined as a person who organizes, operates and assumes the risk for a business venture or other enterprise and includes a “Gig” or even “Micro Gig” worker who earns income providing on-demand work, services or goods through temporary, contract or freelance jobs.  

You could be considering, or participating full-time, part-time or even seasonal activities in this fields.  

PROXY: I appoint the Secretary of the Association in office at any particular time as my proxy to receive notice of and attend all meetings of the members of the Association and vote on my behalf and to otherwise act for me in the same manner and with the same effect as if I were personally present. This proxy shall be valid until revoked. It may be revoked at any time by sending another signed proxy with a later date to the Secretary, notifying the Secretary in writing that you have revoked your proxy or by voting in person at any meeting.
